• Umpire Signup Form - Application form for Menlo-Atherton Little League Umpires. Forms must be received by March 1st. See the Umpires page for more details.

• Volunteer Application Form - All Coaches and Assistant Coaches must fill out this form and submit it along with a copy of their driver's license to their League Commissioner before beginning and practices or workouts.

• Safety Class Handout- All Coaches must download and read this handout on what to do in case of medical emergencies on the field.

• Medical Waiver Form - All players must submit a new, completed Medical Waiver form before starting practice for any MA Little League season or post-season tournament - even if they submitted one for a previous season or event.

• 2009 Fall Ball Registration Form - Application for the 2009 Fall Ball Season. All Forms (including a new Medical Waiver Form and $75/player) must be received by August 30th.
